Wrist Case 2

Clinical History:

F58 with persisting pain & swelling at dorsum of wrist 3 months after arthroscopy.

Imaging:

REVEAL DIAGNOSIS

Diagnosis:

Localised post-surgical arthrofibrosis

Discussion:

Ultrasound showed localised tender, hypoechoic, hyperaemic scarring of the wrist capsule at the arthroscopy portal (asterisk), presumably post-surgical 'arthrofibrosis'. Also note the well-defined fluid-filled defect in SL ligament continuity at the lunate attachment (arrow). This was quite localised and spared the more distal fibres of dorsal band SL ligament (with no dynamic instability being evident on real-time scanning during applied ulnar deviation stress). NOTE:  RAD = radius, S = scaphoid, L = lunate.
